About the role
Work with patients in their own homes to manage medicines following discharge from hospital. The Pharmacy Connecting Care team reviews patients, conducts clinical medicine reviews and carries out interventions to improve pharmaceutical care. Roles include medication education, assessment of adherence, and follow‑up to monitor outcomes.
Work schedule: This post does not routinely include weekends and bank holidays.
Encouraged to undertake the Non‑Medical Prescribing course if not already completed.
Responsibilities
* Contribute to the development and maintenance of policies and guidelines.
* Liaise with all grades of medical and nursing staff over relevant aspects of the pharmacy service and implement necessary service changes.
* Identify, report on, and respond to risk management issues or clinical incidents.
* Generate and review reports to inform the Trust about medication usage, expenditure and safety issues.
* Participate in audits to demonstrate effectiveness or identify development needs of all systems relating to the use of medicines.
* Support Formulary & Drug Usage Audit initiatives in collaboration with the Medicines Information department.
Clinical service commitment consists of visiting patients in their own homes or care homes to conduct clinical medication reviews and improve adherence.
Essential Person Specification Requirements
* Master’s Degree in pharmacy obtained through undergraduate training.
* Registered as a pharmacist with the General Pharmaceutical Council and experience in pharmacy practice obtained through one year pre‑registration training and examination.
* Good written and verbal communication skills.
* Good clinical knowledge.
* Broad experience of clinical working in a number of specialty areas at a developed level.
* Ability to talk to patients, medical and nursing staff of all grades in both primary and secondary care, and present reports to senior hospital staff.
* Standard keyboard skills and familiarity with routine office software (word processing, spreadsheets, PowerPoint).
* Enthusiasm for the development of the practice of Pharmacy.
* Ability to work both independently and within a team.
* Respond positively to service deadlines and possess strong organisational attributes.
* Ability to plan and manage own workload.
* Understanding of and ability to handle confidential and sensitive information.
* Ability to communicate complicated medication issues to patients and carers so they understand and can best use their medicines.
* Ability to communicate and support patients and/or carers with language, sensory or learning difficulties, or who may be dying or distressed.
* Physically able to visit wards and departments across the Trust as required.
* Willing and able to travel throughout the Wakefield District area to cover community services roles.
