Role Overview: As a Community Nurse, you will play a crucial role in delivering holistic care to patients in their homes and community settings. Your responsibilities will include assessing patient needs, creating care plans, administering treatments, and collaborating with a multidisciplinary team to ensure the best possible outcomes for patients. Key Responsibilities:
* Conducting comprehensive assessments of patients’ health needs and developing personalised care plans.
* Administering medications, treatments, wound care, and other nursing interventions in patients’ homes.
* Educating patients and their families on health management, disease prevention, and treatment options.
* Collaborating with physicians, therapists, social workers, and other healthcare professionals to provide coordinated care.
* Monitoring patients’ progress and adjusting care plans as necessary.
* Ensuring accurate and timely documentation of patient interactions and interventions.
* Participating in community outreach programs and health promotion initiatives.
Qualifications:
* Valid NMC Registration
* Experience in community nursing, home health, or a similar setting is preferred.
* Strong clinical assessment and critical thinking skills.
* Excellent communication and interpersonal abilities.
