Band: 7 • Hours: 37.5 per week, Monday to Friday • Location: Based in Epsom with travel between Dorking and Molesey
About the Role
Are you an enthusiastic and motivated pharmacist looking for a flexible opportunity in frailty and community hospital care? We are seeking a Rotational Senior Clinical Pharmacist to provide expert support across a range of community hospital sites.
Responsibilities
* Deliver clinical pharmacy input within community hospital and inpatient frailty services.
* Review and optimise medicines for patients living with frailty and polypharmacy.
* Support discharge planning and safe transitions of care.
* Contribute to service development, assisting senior colleagues with guidelines, policies, and procedures to ensure safe and effective use of medicines.
* Gain valuable experience through wider rotations, developing your clinical, leadership, and managerial skills.
Qualifications
* GPhC-registered pharmacist with strong clinical knowledge and experience, ideally within frailty, older people, or community hospital services.
* Excellent communication and interpersonal skills, with the ability to work collaboratively within an MDT.
* A proactive and flexible approach to service delivery.
* Commitment to evidence-based, patient-centred care.
