Description Support Planner - Urgent Community Response Team Salary: £33,531 - £38,052 per annum Grade D Permanent Contract 37 Hours per Week (Full Time) A Support Planner is required due to the statutory nature of the service and the need to ensure business continuity and collaborative working with health partners to ensure timely and effective support for residents of Thurrock. This post also has links to carrying out statutory duties under various legislations including the Care Act 2014 (carrying out assessment of care and support needs with focus on the person's needs and how they impact on their wellbeing and the outcomes they want to achieve), Mental Health Act 1983 (legislation that covers the assessment, treatment and rights of people with a mental health disorder) and Mental Capacity Act 2005 (statutory framework to empower and protect vulnerable people who are not able to make their own decisions). The postholder will need to ensure that the needs of Thurrock residents are met in a timely manner to prevent a deterioration in their condition that may lead to hospital admission. The postholder will work closely with health colleagues, to facilitate timely hospital discharges with the appropriate care and support in the community. The postholder will need to ensure that carers and families of Thurrock residents with care and support needs are appropriately supported through timely intervention. The postholder will work closely with health and other partners, ensuring Thurrock residents with care and support needs do not get admitted inappropriately/prematurely to hospital/care homes and are discharged from hospital in a safe and timely manner. DBS Check: A Disclosure and Barring Service (DBS) Check at Enhanced Level with Adults Barred List is required for this post.
