Social Worker - Multidisciplinary Team (MDT) - Walsall, UK - £28.40 Hourly
Join us for an exciting three-month locum position as a Social Worker in Walsall, contributing to a dynamic Multidisciplinary Team. Our full-time role runs from 09:00 to 17:30, offering you the chance to employ your skills and experience to positively impact the community. For £28.40 per hour, you will play an essential role in assessing and supporting individuals following their discharge from hospital. This position is exempt from the West Midlands MOC for 12 months, allowing you to focus on delivering outstanding adult social work services.

What you will do
* Complete initial and subsequent reviews of care packages at home and in short-term placements post-hospital discharge.
* Identify the need for occupational therapy, promoting independence with the use of assistive technology and signposting to community resources.
* Determine and coordinate long-term support plans if required.
* Revise support plans to reflect changes in care needs, completing MCAs and risk assessments as necessary.
* Refer individuals to extra care housing or in-house provider services when needed.
