An exciting opportunity has arisen to join the newly transformed Older Adult Community Mental Health Team in BaNES. The individuals who are served by the team are experiencing mental ill health which is having a significant impact upon their well being, and ability to live independently, or are living with dementia, and presenting with high levels of associated behavioural and psychological symptoms. As a support worker in the team, you will be highly motivated and able to work independently, and be responsible for planning, providing and reviewing strength based interventions aimed at supporting individuals well being and independence.
Main duties of the job
To work alongside a multidisciplinary team, under the indirect supervision of registered clinicians.
Key activities will include:
* Forming compassionate, recovery-focused, therapeutic relationships with service users and their carers, supporting them to manage their mental health needs in a community setting.
* Engaging in the full range of ongoing needs assessment processes required by the team, including mental state, dynamic clinical risk, spiritual and cultural needs, and emotional, psychological and physical wellbeing.
* Planning and undertaking defined strengths based interventions and treatments to enable the individual to meet and develop their ongoing daily living skills, to increase confidence and independence in line with agreed person-centered personal well being plans.
* Empowering service users to self-manage aspects of their ongoing treatment, such as medication, diet and lifestyle, physical health and general wellbeing.
* Supporting individuals and their carers to manage transitions in care, and in accommodation.
