Do you want to work in a high-profile programme in Stoke-on-Trent, within a team of experienced and committed professionals to effect system and service change for individuals experiencing multiple disadvantage?
If the answer is yes, then Changing Futures Stoke-on-Trent has an exciting opportunity for you to join its dynamic team as a Senior Social Worker.
In this new position you will support a small team of social workers and lead on an exciting journey within the Adult Social Care team to embed the Care Act principles for a cohort of individuals often excluded from services.
You will advocate for the needs of our clients, and embed a more flexible, adaptable approach to ensure timely Care Act Assessments and a robust co-ordination of care and support. With small caseloads overall, you will work closely with two social work practitioners to develop a consistent approach of support, identifying gaps and barriers in the system, working alongside the Case Manager and Strategic Manager to infuse a well-adapted approach to our clients. Oversight of more complex cases referred to the Multi Agency Resolution Group (MaRG) will be a key role, alongside an innovative approach to supporting swift and flexible solutions to prevent crises via the utilisation of the allocated personal budget to each client.
The Changing Futures programme is a £64 million joint initiative by the Department for Levelling Up, Housing and Communities (DLUHC) and The National Lottery Community Fund, the largest funder of community activity in the UK.
Changing Futures Stoke-on-Trent works in partnership to better support those who experience multiple disadvantage, providing essential support and treatment for vulnerable adults facing a combination of homelessness, substance misuse, poor mental health, domestic abuse, or contact with the criminal justice system.
