We are excited to offer this new opportunity to be involved in the implementation of frailty models within Belford Hospital. This post will be instrumental in developing the model within Belford hospital, working closely with ED and wards medical, nursing and AHP teams to provide rapid assessment and support to older frail patients in the hospital.
Applications are invited from those with enthusiasm to work in a fast paced and changing health and social care system. This new service will deliver frailty assessment with a focus on admission avoidance at A+E where appropriate, as well as setting goals and criteria for discharge as early in the patient journey as possible. With an in depth understanding of community services, you will work in partnership with the MDT throughout Belford as well as the teams in community to link patients to the correct services to achieve Right care, right place, first time.
This service will run 37 hours over 5 days. As a new service, the ability to set up data collection to evaluate the efficacy of the service will be key. A strong communicator who can galvanise the MDT, some of whom may require support to implement your ideas, will be essential skills in this role.
As part of the implementation, you will make recommendations on how the service should develop and, as such, the job role may change as these recommendations take place.
