Provide a highly efficient medicines management service to various wards within the Acute, Medicine, Care of the Elderly, and Women's and Children's departments.
Promote safe, efficient, and cost-effective use of medicines at ward level.
Support the expansion of the medicines management pharmacy technician service across the Trust.
Assist in the induction, training, supervision, and support of new pharmacy staff members.
Train healthcare staff on all aspects of medicines management at ward level.
Participate in medicines management audits.
Complete medicines reconciliations promptly to meet NICE guidelines.
Assess patients' own drugs (PODs) for suitability.
Order 'one stop dispensing' medications timely to support discharge schemes.
Perform final accuracy checks on clinically screened prescriptions for at least eight hours per month in dispensaries or satellite pharmacies to maintain ACPT competency.
Advise patients, representatives, and healthcare professionals on medication issues to optimize patient outcomes.
Provide technical support to satellite pharmacies and main dispensaries.
Maintain own Continuing Professional Development and training.
