Main duties of the job
As a New to Practice (ARRS) Salaried GP, you will work as part of a supportive, experienced team at The Misbourne Practice (one of the 3 practices within The Chalfonts PCN), delivering high-quality primary care to a diverse patient population. Your main responsibilities will include:
* Managing a personal and varied caseload, providing care for patients with acute, chronic, and complex health needs
* Undertaking telephone and face-to-face consultations, including home visits where necessary
* Participating in the duty doctor rota, enhanced access requirements, and contributing to on-the-day access provision
* Liaising effectively with the wider multidisciplinary team to ensure seamless, patient-centred care
* Providing clinical input into practice meetings, audits, and service improvement initiatives
* Contributing to QI projects, teaching opportunities, or leadership development activities
* Participating in regular mentoring sessions
The post will require travel between our two practice sites in Chalfont St Peter and Chalfont St Giles.

The Misbourne Practice is a friendly, well-established training and teaching practice operating across two sites (in Chalfont St Peter and Chalfont St Giles), serving approximately 12,000 patients. Our community includes a high proportion of middle-aged and elderly patients, as well as many young families with children, offering a varied and rewarding clinical experience.
We are proud of our long-standing commitment to education and development, currently focusing on GP training. Our clinical team includes 7 GP Partners, 3 Salaried GPs, and 3 Trainee Doctors, supported by a skilled and dedicated PCN MDT (including Clinical Pharmacists, a Physio, Physician Associate, Pharmacy Technician, Care Coordinator, and Social Prescriber) and administrative and nursing staff.
The Misbourne Practice is part of The Chalfonts Primary Care Network (PCN), working collaboratively with The Allan Practice and The Hall Practice. Together, we serve a combined population of approximately 32,000 patients in the local area.

Job responsibilities
Clinical Duties:
* Provide face-to-face and telephone consultations for routine and urgent care.
* Conduct patient home visits (low frequency)
* Manage patient care in surgery, including investigating and reviewing results, developing care plans, and making referrals to secondary care or other providers.
* Issue and sign repeat prescriptions, ensuring they align with national/local guidelines.
* Administer and handle patient-related queries, test results, referral letters, and other tasks in a timely manner.
* Record clear, contemporaneous consultation notes using EMIS.
* Ensure safe, evidence-based, and cost-effective care, in line with Practice disease management protocols and NICE guidance.
* Independently prescribe medications within your scope of practice.
* Review and manage patient medications following appropriate policies and local clinical guidelines.
* Utilise other available clinical and support staff in delivering holistic and patient-centred care.
* Actively participate in MDT and care conference meetings, as required.
* Participate in on-call and enhanced access duties on a pro-rata basis, if required.
Health Promotion and Patient Support:
* Regularly screen for lifestyle factors and support patients in adopting health promotion strategies and self-care principles.
* Contribute to the delivery of anticipatory care plans.
Professional Development:
* Maintain awareness of clinical practice developments.
* Attend and participate in meetings, PLTs, and mentor sessions as required.
* Participate in an annual performance review and take responsibility for personal development.
* Engage in mandatory training and attend courses as needed.
Team Collaboration:
* Provide clinical support and guidance to both clinical and administrative staff to maintain high care standards.

Qualifications
Essential
* Newly qualified GP, within 2 years and not having had a substantive post
* Registered with the General Medical Council (GMC) and licensed to practice
* Membership of appropriate Medical Organisation (MDU, MPS, etc)
* Inclusion in NHS England /Performers List of Medical Practitioners
Experience
Essential
* Experience of working in urgent care and/or a primary care environment
* Experience of dealing with a range of clinical conditions
* Experience of providing home visits
* Proven ability to evaluate the safety and effectiveness of own clinical practice
* Understanding of evidence-based practice
* Experience working autonomously, with some support and supervision

* Experience in Triage
