I am currently representing Westminster Council, who are offering an initial temporary contract for a period of 3-6 months with a view to be extended or offered a permanent position for the right candidate at a rate which is negotiable dependent upon experience
We are looking for a Mental Health Social Worker this role will be: SW1V 1DX
The right candidate will:
Westminster City Council is seeking an experienced and motivated Mental Health Social Worker to join our South Locality Adult Social Care Mental Health Team. This is an opportunity to work within a dynamic, multi-disciplinary environment, delivering high-quality social care support to adults experiencing mental ill health across our community.
* Participating in the daily duty rota and handling incoming calls and referrals
* Undertaking Care Act strengths-based assessments • Completing Care Act care and support plans and conducting reviews
* Applying for funding for placements when required • Working jointly with health colleagues to develop Section 117 aftercare plans
* Attending MDT ward rounds and community MDT meetings
* Planning safe and timely discharges for individuals in acute or psychiatric hospital settings
* Acting as a Lead Enquiry Officer (LEO) for safeguarding enquiries
* Carrying out Carers Assessments under the Care Act
We require the following:
* Social Work England registration (required)
* Experience working within a community mental health team
* Strong understanding of the Care Act, Mental Capacity Act and safeguarding processes
