Job summary
ThePCN Advanced Paramedic Practitioner role at Farnham Integrated Care Services(FICS), Farnham Centre for Health, will be a registered Paramedic with postgraduate qualifications, including the Independent and Supplementary Prescriberqualification and HCPC registration, acting within their professionalboundaries.
Thisrole will work as part of a multi-disciplinary team of AHPs/GPs/HCAs etc,providing care for the presenting patients within our Urgent Same Day CareService at FICS. An established integrated service supporting the GP Practicesof Farnham.
Frominitial history taking, clinical assessment, diagnosis, treatment andevaluation of their care. They will demonstrate safe, clinical decision-makingand expert care for patients within the service
Main duties of the job
The PCN Advanced Paramedic Practitioner is expected to attend to patients within the defined list of conditions, including assessment, diagnosis and treatment to deliver quality patient services in conjunction with the daily duty doctor.
The PCN APP will work collaboratively with the multi-disciplinary team to meet the needs of patients, supporting the delivery as per policy and procedures. There is an expectation that training across the service is maintained to an expected level with use of appropriate equipment and facilities as directed by the post holder who is an autonomous allied healthcare professional who will be part of PCN support to practices while working at FICS.
The post holder will use advanced clinical skills to provide education to service users, promoting selfcare and empowering them to make informed choices about their treatment.
The PCN APP will actively support integrated ways of working to support standardization for efficiency purposes.

Job responsibilities
Thefollowing are the core responsibilities of the PCN Advanced ParamedicPractitioner
7. Alongside the multidisciplinary team, to provide a first point of contact for patients presenting with undifferentiated, undiagnosed problems. Making use of skills in history taking, physical examination, problem-solving and clinical decision-making to establish a diagnosis and management plan
8. To provide assessment, treatment and diagnosis at point of first contact by attending to patients within our Same Day Service at FICS.
9. To make professionally autonomous decisions for which he/she is accountable
10. To refer patients to an alternative care setting or treat and discharge as appropriate
11. To instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports
12. Generate EPS and FP10 prescriptions within scope of practice, considering local and national guidelines.
13. To ensure that professional standards are maintained and within the guidance provided by the Department of Health, the Health and Care Professions Council (HCPC) and the College of Paramedics (COP)
14. To support the development of excellent relationships across the PCN to enable collaboration for better patient outcomes
15. To support network implementation of agreed service changes and pathways, working closely with other staff members, the wider PCN and the commissioner to develop, support and deliver local improvement programmes aligned to national and local priorities.
16. To contribute to the service achieving its quality targets to sustain high standards of patient care and service delivery
17. To participate in identification of community health needs and develop patient/family-centred strategies to address them
18. To help develop and set up new patient services and participate in initiatives to improve existing patient services
19. To support in the delivery of enhanced services and other service requirements on behalf of the PCN
20. To participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through clinical incidents and near-miss events
21. To undertake all mandatory training and induction programmes
22. To contribute to and embrace the spectrum of clinical governance
23. To attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed
24. To provide effective visible leadership that fosters a culture of professionalism, compassion, excellence and the development of effective team working
25. To develop yourself and the role through participation in training and service redesign activities
26. To contribute to public health campaigns ( flu clinics) through advice or direct care.

28. Inaddition to the primary responsibilities, the PCN Advanced ParamedicPractitioner may be requested to:
29. Support delivery of QOF, incentive schemes, PCN IIF targets, QIPP and other quality or cost effectiveness initiatives
30. Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner

34. Minimum of two years working as an Advanced Paramedic
35. An appreciation of the nature of GPs and an urgent Care Centre.
36. An appreciation of the nature of Primary Care prescribing, concepts of rational prescribing and strategies for improving prescribing
37. Awareness of telephone triage assessment
38. Management of common minor injuries/illnesses
39. Clinical examination skills
40. Knowledge of needs of patients with long-term conditions
41. Management of long term (chronic) conditions
42. Understanding of the mentorship process
43. An appreciation of the new NHS landscape including the relationships between FICS, PCNs and the commissioners
44. Experience of clinical governance including audit
