GLL is looking for a Exercise Referral Instructor to work across the sport and leisure facilities in Reading. The Exercise Referral Instructor role is responsible for prescribing suitable physical activity and supporting behaviour change for Medically Referred clients. The Health Referral Facilitator role will include setting up and undertaking group and individual training sessions along with general day to day operation of the GP Referral programme including referral processing, accurate record keeping and data processing.
This role is specifically to deliver on Falls Prevention community-based programme and will involve travel into community venues and Leisure Centres across Reading. A Postural Stability Instructor (PSI) qualification is desirable but not essential. PSI Training will be provided to the successful candidate.
The Exercise Referral Instructor (Health Referral Facilitator) is responsible to the Healthwise Coordinator / Health Manager.
What you’ll do:
1. To ensure the smooth day-to-day running of the Physical Activity Referral Scheme (PARS), maximising effectiveness of all available resources.
2. Carry out health and lifestyle assessments with referral patients and develop safe and appropriate programmes of physical activity / exercise.
3. Act as first point of contact for patients referred onto the scheme
4. To provide individuals with evidence-based, relevant and culturally sensitive information regarding a healthy lifestyle.
5. To constructively interact with different people in a variety of areas within the operational team, wider health teams and GLL support services.

What you need:
6. GP Referral qualification (Level 3)
7. A recognised Fitness instructor or Personal training qualification (Level 3)
8. Experience of pre-screening / assessing patients for suitability to physical activity / exercise programme
9. Experience of monitoring and progressing individuals through an exercise programme
10. A good understanding of the health and fitness industry
11. Ability to motivate individuals and support behaviour change
12. Good time management and punctuality
