An exciting opportunity has arisen to join the heart failure team. You will be providing integrated heart failure specialist care across the community and within hospital, working to diagnose, treat effectively and reduce hospital admissions amongst our patient population.

EDUCATION, QUALIFICATIONS & TRAINING
Essential criteria
1. Registered General Nurse - Currently registered with the Nursing & Midwifery Council
2. Cardiology experience
3. Post registration Cardiac Specialist Heart Failure qualification e.g. Caledonian
Desirable criteria
4. Working towards a degree
5. Independent Non-Medical Prescribing Qualification.
6. Advanced Life Support Course
7. Substantial experience in delivering community based cardiac care
8. Assessment and diagnostic experience and qualification or willing to work towards
9. Independent Non-Medical Prescribing Qualification (or willing to work towards)
EXPERIENCE
Essential criteria
10. Evidence of continued professional development related to cardiology. Experience of managing Heart Failure conditions
11. Evidence of ability to measure and evaluate outcomes of audits of cardiology patients
12. A significant contribution to practice and service development.
Desirable criteria
13. Evidence of effective budget and resource management
14. Ability to initiate, sustain and evaluate change
15. Experience in delivering formal and informal education and teaching self management to complex patients / patient groups

16. Knowledge of Cardiac Heart Failure Nursing and service delivery requirements
17. Excellent communication skills: interpersonal, written and verbal
18. Experience of implementing change
19. In depth knowledge of the clinical governance agenda including clinical risk and safety.
20. Able to lead, manage and motivate others in complex problem solving and assessment skills
21. Well developed IT skills/ word processing/ data collection/ internet skills
