As a Transition Facilitator, you will be part of a team who support and work with children and young people aged 10-25 years. Your work promotes independence of the individual, with the aim of being able to reach their full potential within their home and wider community. Some of the children and young people you work with may have special educational needs and disabilities (SEND).

As a Transition Facilitator, you will be part of a team who support and work with children and young people aged 10-25 years. Your work promotes independence of the individual, with the aim of being able to reach their full potential within their home and wider community. Some of the children and young people you work with may have special educational needs and disabilities (SEND).
Based in Badminton Road, the team sits within South Gloucestershire’s 0-25 disabilities service. They have a focus on early interventions and supporting key transitions with those referred to the service.
What You Will Be Doing
* Daily, you will assess, monitor, evaluate and review cases and care plans against the identified outcomes.
* It will be your responsibility to maintain confidential reports and case notes and record care plans and risk assessments in accordance with professional and council guidelines.
* Regularly, you will provide advice, support and guidance to children, young people and their families in a strength-based person-centred approach.
* Working with families, you will promote the Local Offer and liaise with their networks which could include social workers, health professionals, schools/ colleges and colleagues in our EHCP team.
* You are responsible for liaising and working with external providers e.g. health agencies, housing and emergency services, ensuring your communication is effective.
* It will be key for you to act in accordance with safeguarding procedures in keeping vulnerable people safe.
What We Need From You
* It is essential you hold or be working towards a professional qualification in any discipline eg NVQ 3 or equivalent.
* Given the nature of the role, you need to have experience of working in similar service areas with children and young people, especially with those who have complex needs and challenging behaviours.
* Ideally, you will demonstrate knowledge of relevant legislation, regulations and codes of practice, for example Safeguarding, Mental Capacity/Deprivation of Liberties (DoL’s); The Local Offer.
What You Need To Know
* This a permanent, full-time role working 37 hours per week, Monday to Friday.
* You will be expected to travel throughout the authority and surrounding area so you must have a full, valid driving licence with regular access to a vehicle or have an alternative means of travel.
* Business insurance will need to be added to your current insurance policy.
* Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) for both Children and Adults is obtained for this post.
Interviews will be held on Thursday 7th August 2025.
