Overall job purpose

The post holder will provide Occupational Therapy in Llesiant Meddwl Mon in accordance with statutory and best practice requirements. They will apply specialist knowledge of Occupational Therapy and expertise that is underpinned by theory, evidenced based practice and continued professional development. They will commit to work towards the Approved Mental Health Professional qualification and enrol at the earliest opportunity of being appointed into the post.

They will work as part of a team with a range of other professionals. Working in a changing environment, the post holder will assist the Department in meeting its responsibility to implement the Social Services and Wellbeing Wales Act and achieving the aim of closer working relationships with Health and other partners to deliver improved outcomes across the social care sector.

They will strive to maximise the independence, wellbeing, and safety of adults and to improve their mental wellbeing. The post holder will do this by undertaking assessments of need using the Department's procedures, and where required will undertake separate assessments of the needs of the person's carer(s). They will

develop and design outcomes and support plans, and other appropriate plans, and recommend any external support within budgets, ensuring the best use of resources, and ensuring necessary approval when required e.g Resource Panel Applications.

They will plan creatively with individuals, maximising assets, reducing risks, and reducing dependency on long term service provision. This will include:

* Monitor and evaluate the ongoing effectiveness of the OT intervention plan: through a reflective model demonstrating knowledge to measure and reflect critically on practice and the skills in relation to analysis of outcomes.
* Care Management Role/Key Worker Role: Take responsibility for assessing needs and manage case load
* Crisis Duty Role: To be responsible for providing a rapid response service for individuals based on their assessed level of need and eligibility to the service which consists of screening, assessment, intervention planning, resolution and or/referral.
* Consultancy Role: To be responsible for advising and consulting with multidisciplinary team on a range of issues. This may include supporting wider teams within Adult Services in relation to assessment and interventions.
