We have an exciting opportunity for a Care Coordinator to join our domiciliary and extra care services across Ellesmere Port and Chester. As a Care Coordinator, you'll play a vital role in ensuring the smooth delivery of high-quality, person-centred care. You'll work closely with care staff, the people we support, families and healthcare professionals to ensure services are safe, effective and tailored to individual needs. This is primarily an office-based position with occasional field support when required. You'll work on a structured shift rota covering 7am3pm and 3pm11pm shifts, including alternate weekends and on-call duties.
