Role: Social Worker – Adult Mental Health

Purpose of Role
Social Personnel is advertising a Social Worker role for Stockton-on-Tees Borough Council. We are seeking an experienced and motivated Social Worker to join the Adult Mental Health team. You will play a key role in safeguarding, supporting, and empowering adults with mental health needs, ensuring they receive personalised care and protection in line with statutory duties.
Key Responsibilities
* Conduct strength-based assessments under the Care Act 2014, delivering tailored support plans.
* Work collaboratively with partner agencies to ensure coordinated and effective interventions.
* Support individuals detained under the Mental Health Act 1983, ensuring safe discharge and robust Section 117 aftercare plans.
* Assess eligibility for Continuing Health Care (CHC) and lead Decision Support Tool meetings.
* Carry out mental capacity assessments and make best interest decisions under the Mental Capacity Act 2005.
* Engage with the Court of Protection to uphold individuals’ rights and safeguard wellbeing.
Essential Skills & Experience
* Minimum of two years’ experience working with adults whose primary need is mental health.
* Qualified Social Worker registered with Social Work England.
* Strong knowledge of relevant legislation including the Care Act 2014, Mental Health Act 1983, and Mental Capacity Act 2005.
* Excellent communication, assessment, and multi-agency working skills.
Desirable
* Experience of working within local authority adult social care services.
* Knowledge of local health and social care pathways across Stockton-on-Tees.
