This job opportunity will see you work closely with multi-organisation partners to improve the patient experience and timeliness to the most appropriate health and care services.
Main duties of the job
We are looking to recruit an experienced clinician into our friendly team as the service continues to grow and develop. If you have experience of working in acute, community or rehab services dealing with complex challenges and are keen to do something different in a highly performing, stimulating and fast-paced environment then this may be your opportunity.
You will be fully supported by the team and service manager as you grow your knowledge of clinical services within West Hertfordshire and beyond to ensure patients receive the most appropriate care in a timely manner.

6. Registered Nurse with current UK NMC registration

7. Evidence of continued professional development and post registration training

8. Understanding of working in a rehabilitation unit or intermediate care setting
9. Knowledge of negotiating and liaising within a multi-disciplinary team
10. Experience of working with people with complex needs
11. Experience of supervising junior staff and coordinating a team on a day to day, regular basis
12. Experience of evidence-based care
13. Experience of undertaking comprehensive health needs assessment and writing care plans
