Salary: £39,144 - £41,110 per annum (Inclusive of 5% Market Supplement)

We are looking for a Social Worker to join our team and provide professional growth opportunities and flexibility for work‑life balance. This role collaborates with health colleagues in a multi‑agency integrated service focused on Health and Social Care. The aim is to reduce unnecessary hospital admissions, support carers, and promote independence by assessing people in their own homes and assisting them to maximise independent living.
Responsibilities
* Collaborate with health and A&E departments to provide community support before hospital transfer.
* Assist the manager in delivering a holistic service, ensuring the safety of vulnerable adults.
* Facilitate prompt assistance for adults and their caregivers during crises such as falls or caregiver emergencies.
* Assess needs for eligibility, devise care plans, and achieve or maintain well‑being outcomes.
Qualifications
* Registered with Social Work England.
* Strong interpersonal and communication skills.
* Updated knowledge of relevant legislation and an in‑depth understanding of adult social care legislation.
* Motivated, organised, energetic and creative.
We offer a 5% market supplement payment, subject to annual review.
