* To work as part of a team supporting people with multiple complex mental health needs who are also experiencing homelessness.
* Reporting directly to the team manager and under the professional supervision of the Peer Support Worker Lead.
* To work across systems within GHC and across organisation, including Housing, Adult Social Care (ASC), Change Grow Live (CGL) substance misuse service, and other relevant partners.
* To work in unconventional clinical environments (e.g. rough sleeper sites, supported housing, partner agency units, in the community etc) to deliver support to individuals with complex emotional needs.
* To work with direct partners, team, senior management, commissioners, and partners to implement NICE guidelines NG214 Integrated Health and Social Care for people experiencing homelessness' within the Trust, the team and wider services.
* The role of peer support worker has been developed specifically for people who have lived experience of complex needs and homelessness.
* The successful candidate would be a highly valued member of the multi-disciplinary team, the PSW during the development phase will offer limited support and practical assistance to service users in order for them to regain control over their lives and their own unique recovery process.
