Job summary

Be an active member of the clinical team, providing a comprehensive therapy service to residents of West Hertfordshire referred into the service. Work within professional standards and clinical guidelines, promote best practice and undertake all aspects of clinical duties as an autonomous practitioner including assessing and treating own caseload of patients/clients and maintaining professional documentation. Build and maintain key relationships with referrers to the service, to facilitate appropriate referrals and an understanding of the role of the Physiotherapist and the Multi-professional team. Be responsible for the supervision of junior staff, rehabilitation assistants and Physiotherapy students. Support self and service development through active participation in Continuing Professional Development (CPD) and by participating in any audit or research being undertaken by the service.

Main duties of the job

* Undertake specialised uni-professional and multi-professional assessments.
* Develop and implement treatment plans for clients presenting with an exacerbation of a frailty related condition and likely multiple comorbidities including neurological conditions, musculoskeletal conditions, osteoporosis, learning disability and/or mental health diagnosis.
* Participate in the Assessment Process when appropriate, performing overview assessments and complex specialist assessments as appropriate; including the ordering of moving and handling equipment relevant to rehabilitation need.
* Communicate relevant medical information, assessment details, advice and recommendations across agencies and with clients in compliance with Trust Guidelines for sharing of information and informed consent. Agencies include General Practitioners, Social Services, Healthcare professionals, voluntary agencies, and residential homes.
* Be responsible for gaining valid, informed consent for assessment, treatment interventions and sharing of information in compliance with legal and professional guidelines. Interpret and follow these guidelines in circumstances where clients do not have capacity to provide informed consent.

Person Specification

Education/Qualification

* Diploma/degree in Physiotherapy
* Current HPC registration
* Evidence of up to date CPD portfolio
* Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation, Neurology, Orthopaedics, Musculoskeletal Outpatients, Older People Rehabilitation
* Member of Professional Body

Experience

* Essential: Experience post registration as a Band 5 physiotherapist
* Experience working at a Junior level in a wide range of specialised clinical areas including outpatients, neurology, rehabilitation, orthopedics and respiratory

Skills & Knowledge

* Essential: Skills in the assessment and treatment of adults with complex multi-pathologies
* Knowledge and understanding of complex medical conditions including multipathologies and falls
* Knowledge of role of Allied Health Professionals and interdisciplinary team working
* Ability to apply core Physiotherapy skills across a range of clinical conditions within a client centered, goal setting
* Knowledge of outcome measures and evidence-based practice
* Ability to problem solve, prioritise and demonstrate clinical reasoning
* Good communication and report writing skills, in both verbal and written format
* Knowledge and understanding of the role of Social Services and voluntary agencies

Desirable

* Knowledge and experience in research and audit
* Knowledge and understanding of National Service Framework for Older People
* Knowledge and understanding of NICE and CSP/COT Fallers Guidelines
* To have a full UK drivers' license
* Car owner
