Community Palliative Clinical Nurse Specialist

30 hours per week: Monday to Thursday 0830hrs -1700hrs.Salary: £43,300 per year, pro rata. Actual salary: £34,700 per year.

Main duties of the job

Working as part of a community team covering the whole of North Devon, you'll collaborate closely with colleagues across a range of teams and organisations within the locality to support people in their own homes.You'll be able to demonstrate experience in managing a caseload within a community setting and have a strong foundation in symptom management. While working within a close-knit and supportive team, you'll also show confidence and autonomy in your practice. A commitment to person-centred care and evidence-based practice is essential.

If you're not already a non-medical prescriber, full support will be provided to undertake the required training and achieve registration.

For candidates without previous specialist palliative care experience, a tailored induction and training programme will be provided. We actively support continued professional development and encourage all team members to pursue learning opportunities that support their growth and development.

Job responsibilities

* Working with the community team, to help deliver flexible, responsive, patient-led,outstanding care to people in their own homes.
* Liaise with GPs and surgeries to provide best patient led care jointly.
* Carrying a clinical caseload and use expertise to guide and support others in themanagement of complex palliative care needs.
* Contributing to the governance and monitoring of our services through compliance with andinvolvement in the regular delivery of audit.
* Directly managing your own caseload, assessing complex, specialist palliative care needs,planning and implementing individualised programmes of care and evaluating effectiveness, impact and outcomes with the patient, those close to them and other key colleaguesinvolved in the patients care.
* Promoting an environment and culture where patient led, individualised care is central toour way of working.
* Work with the team to develop skills of enquiry in order that we always look for ways todevelop and improve services to achieve the best outcomes.
* Promoting and facilitating effective multidisciplinary working within the hospice and withcolleagues in primary and secondary care to ensure patients receive coordinated andseamless care.
* Working with the team to define and agree standards of nursing care and practice to ensureour care is always safe.
* Maintaining accurate and contemporaneous records of care in line with policy and legalrequirements.Ensuring confidentiality is maintained at all times.
* Supporting the team to understand their role in Safeguarding including recognising issues,reporting concerns and being involved in investigations.
* Supporting staff to report concerns, incidents, learning points, issues for awareness andsafeguarding concerns. Develop action and learning points and investigate where required.
* Acting always to promote and facilitate patients and their families to receive outstandingcare and to listen to their feedback.
* As a CNS, you will be involved in the facilitation of learning and development and supportthe education lead.Providing mentorship and supervision to designated staff as required.
* Your own continued professional development and ensure you are updated on currentresearch, clinical practice and professional matters.
* Ensuring that you follow good infection control practice at all times and that you are familiarwith infection control policies, procedures, and guidance relevant to your area of work.
* Understanding your role in the safeguarding of adults and children by being able to identifyrisks, and recognise signs of harm, abuse or neglect including how to report concerns andtake immediate action to protect a person at risk of harm or abuse.


Person Specification


Qualifications

* Current NMC Registration as a Registered Nurse
* Post registration qualification at degree level in palliative care is desirable.
* Evidence of continued professional development
* NMP or willingness to gain the qualification


Experience

* Significant post registration experience in end of life care, palliative care or a related field
* Experience of working in the community nursing setting
