An exciting opportunity has arisen to join our busy GP practice based at Yarnspinners Primary Health Care Centre in Nelson
The role of the Care Navigator is to be the first point of contact for our patients and supporting them to ensure their needs are met by the most appropriate member of our multi professional team. Receiving patients into the practice and signposting to the most appropriate pathway.
The role is central to the success of the whole team playing an important part in the whole system multi-disciplinary model.
In addition the successful candidates will provide general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person, in writing or via the telephone.
A high degree of accuracy is required in the role as is the ability to work as part of a wider team and on own initiative within practice policy and frameworks.
Main duties of the job
Provide a comprehensive reception service for patients and visitors to the practice, responding in a timely and efficient manner to both telephone and face to face service users
Register patients on the electronic patients system (EMIS) and update these records as appropriate
Understand and maintain confidentiality at all times

Person specification
Qualifications
* English and Mathematics GCSE level 2 or above or equivalent
Experience
* Experience of the receptionist role, greeting patients in a friendly and professional manner
* Ability to manage a high volume of calls whilst maintaining a calm and professional attitude
* Experience of administrative and clerical tasks
* Experience of working in General Practice or similar health care setting
* Experience of using electronic patient records - EMIS
Skills
* Ability to communicate successfully - both verbal and written across all levels
* Good working knowledge of MS Office able to use Word, Excel and Outlook to a high standard
* Able to work as part of a multi disciplinary team
* Ability to work on own initiative with minimal supervision
* Good time management and organisational skills
Personal Attributes
* Positive approach to work with the ability to problem solve
* Willing to undertake any training and development as required by the role
