The Colne Practice is looking to recruit an advanced nurse practitioner on a part time basis for 3 days per week, who is competent in practice using their expert clinical knowledge and skills. You will have the freedom and authority to act and make clinical decisions in the assessment, diagnosis and treatment of patients.,
* Managing patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice.
* Be an extended and supplementary prescriber, adhering to extant guidance.
* Requesting and interpreting pathology services as necessary.
* Maintaining accurate clinical records in conjunction with extant legislation.
* Ensuring SNOWMED codes are used effectively.
* Supporting the team in dealing with clinical emergencies.
* Contributing to practice targets including QOF and IIF.
* Chaperoning patients where necessary.
* Undertaking consultations face to face/telephone/video call and home visits.
* Being responsible for the implementation of an effective immunisation programme.
The Colne Practice is a friendly GP practice with a list size of approximately 10,500 patients. Our team comprises 7 GP partners, 2 practice nurses, 1 HCA, 2 clinical pharmacists and reception and admin teams. We have on site staff parking.
