Job summary
An exciting opportunity has arisen for an enthusiastic and experienced AHP to join the Homefirst team in Central Cornwall. This position is open to Occupational Therapists who have experience working in the community and wish to develop their assessment, care planning, and reablement skills whilst being part of a dynamic, progressive, integrated multiagency, multidisciplinary team.
The Homefirst team is part of the integrated community team in Central Cornwall, comprising registered and non-registered nursing, therapy, and administrative staff working collaboratively with partner organizations to support the intermediate care needs of our local population.
This is a community-based role that requires frequent travel to various locations throughout the county, including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.

Main duties of the job
1. Assess, plan, and implement patient care.
2. Interpret information relevant to the patient's condition/progress, identify problems and needs, and set realistic goals.
3. Maintain excellent organizational skills with a broad knowledge base and experience in older person's services.
We offer a supportive working environment to develop your clinical and interpersonal skills, a structured in-service training programme, appraisal, and supervision system. CPD is actively supported through regular supervision and staff training.
The team is based at St. Austell Hospital but covers the entire geographical area of Central Cornwall. A UK driving license and access to a vehicle for work use are essential.
This is a full-time post, with days planned over a 7-day rota, involving regular weekend work. The service hours are 8 am to 8 pm, with assessors working 8 am to 6 pm.

Person Specification
Qualifications and Experience (Essential)
* Degree or diploma in Occupational Therapy.
* HCPC registered, NMC Registered.
* Evidence of postgraduate courses and CPD supporting practice, equivalent to degree level.
* Substantial post-registration clinical experience in a similar role.
Skills and Aptitude (Essential)
* Teaching, assessing, coaching, and mentoring skills to facilitate effective learning environments for staff and students.
