We are seeking a motivated and patient-focused Pharmacy Technician to join our friendly and supportive GP practice team. Working alongside GPs, clinical pharmacists, nurses, and administrative staff, the successful candidate will play an important role in supporting the safe, effective, and efficient use of medicines within the practice.

The remuneration for this post is open for negotiation depending on experience and the correct candidate, we are more than happy to open discussions for the right person to join our team.
Job Responsibilities
* Support safe and effective repeat prescribing systems within the practice.
* Carry out medicines reconciliation following hospital discharge, outpatient appointments and care transfers.
* Investigate and resolve routine medication queries from patients, pharmacies and other healthcare professionals.
* Liaise with community pharmacies, care homes and hospital teams regarding prescription and medication issues.
* Promote and support electronic repeat dispensing (eRD) and online prescription ordering.
* Assist with identifying medication synchronisation opportunities to improve patient care and reduce waste.
* Support prescription administration and workflow management to ensure timely processing.
* Ensure accurate coding, updating and maintenance of patient medication records on the clinical system.
* Assist with prescribing audits, medicines safety searches and quality improvement projects.
* Support the implementation of prescribing policies, local formularies and medicines management guidance.
* Escalate clinical concerns or complex medication queries appropriately to the Clinical Pharmacist or GP.
* Maintain confidentiality and comply with information governance requirements at all times.
* Support medicines reconciliation following hospital discharge and outpatient appointments.
* Identify patients requiring medication reviews and assist with recalls.
* Work with GPs and clinical pharmacists to improve prescribing processes and patient outcomes.
* Provide advice and support to patients regarding medication queries where appropriate.
* Assist with prescription management, audits, and quality improvement initiatives.
* Ensure accurate coding and maintenance of patient records within the clinical system.
* Support the implementation of national and local prescribing guidance.
* Liaise effectively with community pharmacies, hospitals, care homes, and other healthcare professionals.

Experience
* Previous experience working in general practice or primary care.
* Experience using EMIS, SystmOne, or other GP clinical systems.
* Understanding of QOF, medicines optimisation, and NHS prescribing guidance.
* Completion of CPPE Primary Care Pharmacy Education pathways or equivalent.
