Job Summary
We have a very rare opportunity to join our North and East Integrated Transfer of Care Hub (iToCH) as an administrator supporting the processing of referrals into the North and East community service.
The Administrator will review patient flow blockages, highlight issues to Team Managers, and maintain accurate, timely feedback to referrers. The role involves liaising with multi-agencies for patient flow coordination. Excellent communication and people skills are essential, along with the ability to work under pressure as part of our local point of access team.
This is a part-time position, covering hours from 8 am to 8 pm, 7 days a week, including bank holidays, shared with another team administrator.
Main Duties
The Admin Support Worker collaborates with nurses, social workers, therapists, and other staff to process and manage patient referrals through the Transfer of Care Hub and local point of access.
The role includes monitoring referrals on the electronic system, triaging in partnership with the multi-professional team, and addressing flow blockages by informing managers and liaising with agencies for patient movement.

Qualifications (Essential)
* NVQ 3 in Administration or equivalent
* Experience in a community setting
* GCSE English & Maths or equivalent
Skills and Aptitude (Essential)
* Ability to prioritize workload and work independently with accuracy
* Experience in healthcare office environment
* Proficiency in Microsoft Office, Excel, PowerPoint
* Calm under pressure, self-motivated
