We are seeking a mission-driven Family Nurse Practitioner to join our Primary Care Department at the Harbor Community Health Center - Plymouth.

Role:
The Nurse Practitioner provides comprehensive care including routine health maintenance, management of chronic and acute conditions, tailored to the patient population. The position is full-time, potentially including evening shifts (until 8 PM) and Saturday rotations (8 AM - Noon) depending on clinic needs.
Responsibilities:
1. Serve as a primary care provider for a patient panel.
2. Manage outpatient visits, communicate with patients, coordinate inpatient care, and facilitate referrals for specialty care and patient education.
3. Conduct health maintenance activities, including history and physicals, screenings, and health education.
4. Provide care for acute illnesses and manage chronic conditions, including patient education on self-management.
5. Participate in an on-call system and evening/weekend/holiday clinics.
6. Engage in meetings, trainings, and quality improvement initiatives.
7. Support managed care plans through patient education, appropriate referrals, and cost-effective decision-making.
8. Accurately document in EMR, including progress notes, lab follow-ups, and medication management.
9. Strive to meet or exceed budgeted visits and improve care delivery.
Requirements:
* Graduation from an accredited Nurse Practitioner program in Family Practice
* Current MA NP License, Board Certification, DEA, MCSR, and BLS certification
* Proficiency in MS Office and experience with EMR systems (EPIC/OCHIN preferred)
* 2-5 years of NP experience; bilingual skills in Portuguese or Spanish are a plus
* Experience with community health center populations is preferred
