Clinical responsibilities: Assess the healthcare needs of complex patients with both defined and undifferentiated or undiagnosed problems, carrying out holistic face-to-face patient assessments and developing comprehensive management plans based on Comprehensive Geriatric Assessments (CGAs) Manage patient risk effectively and safely, including high levels of complexity and uncertainty Utilise other clinical staff appropriately to ensure safe and effective patient care, demonstrating awareness of your own limitations and the confidence to escalate when required Contribute to the clinical management of patients as appropriate, including initiating investigations, reviewing results, liaising with secondary care, and making referrals to other providers and services as appropriate including initiation of effective emergency care Contribute to Advance Care Planning and Treatment Escalation Plans through completion of CGAs & ReSPECT forms, including best interest discussions and decisions where required Contribute to the triage of referrals to the Virtual Frailty Ward Record clear and contemporaneous computerised consultation notes to agreed standards Compile and issue computer-generated acute and repeat prescriptions Prescribe generically whenever appropriately and in accordance with NICE guidance and Good Prescribing Practice (as defined by British National Formulary guidance) Provide information and advice on prescribed and over-the-counter medication on medication regimens, side-effects and interactions Work with and support other members of the MDT, including but not limited to Advanced Complex Care Managers, Nursing Associates, Specialist Pharmacists, Physiotherapists, Urgent Care Response Paramedics and other Frailty GPs. Promote the application of standards and codes of Practice set by the Nursing and Midwifery Council or Health and Care Professions Council, Department of Health, British Geriatrics Society and local NHS trust Participate in national frailty training and maintain a high volume of CPD in Frailty-related conditions, with the aim of obtaining the Diploma in Geriatric Medicine
