Join our dedicated Community Response Team as a Reablement Team Leader.
As a Reablement Team Leader, you’ll support adults across Hampshire who need extra care and assistance. Our Community Response Service focuses on short-term assessment and reablement for those with various short and long-term conditions and frailties, including those transitioning from hospital care. We are dedicated to promoting independence and enhancing the quality of life for our service users, helping them stay at home longer.

Our Service runs 7 days a week, therefore you’ll be required to follow a 4-week shift pattern across the week. This includes early, day and late shifts, in addition to weekends, including bank holidays, on a rotational 2:4 basis, for which enhancements are paid.
The current Reablement Team Leader shift patterns are subject to change following an upcoming organisational restructure.
