About the role:
1. Work with an identified number of clients who have repeated, prolonged and/or complex experiences of rough sleeping in Brighton & Hove.
2. To identify and establish contact with long-term rough sleepers, including those unwilling to engage with services, providing assertive outreach
3. Proactively ensure that service users have access to ongoing services such as healthcare, recovery and social care.
4. Work closely with all partner services in Brighton and Hove, sharing appropriate information and ensuring that action plans are carried out
About you:
5. Understanding of the needs of people who are rough sleeping or insecurely housed
6. Understanding the importance of multi-agency working
7. Be an excellent communicator, both verbal and written with good IT skills
8. Proactive and able to work on own initiative with excellent time management and prioritising skills
9. Have a good working knowledge of mental health interventions, services, and good practice
10. Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
11. Ability to manage change successfully in a way that prioritises the needs of service users
12. Understand the importance of information governance processes and commit to follow and apply all necessary safeguards
13. Experience of, and a commitment to, working positively in partnership with a range of statutory and voluntary agencies
