Apollo PCN, based in North Warwickshire, is looking for an enthusiastic and proactive Care Coordinator to join their patient-focused, forward-thinking team. The successful candidate will work alongside our clinical and non‑clinical teams to offer a friendly, caring and supportive experience for patients.
Main Duties
Use EMIS and Docman to manage PCN‑related projects.
Work closely with primary care teams, community organisations and other healthcare professionals to enhance engagement, quality of life and well‑being.
Participate in PCN projects for long‑term conditions, frailty and other health needs.
Hold care coordinator phone and take patient queries.
Carry out other administrative and PA duties as reasonably required.
Job Responsibilities
Report to the Clinical Director / PCN Manager and work with up to five practices.
Support PCN practices to deliver on QOF, KPIs and locally commissioned enhanced services.
Travel to other practices within the Nuneaton and Coleshill areas as required.
Identify opportunities and gaps in the service, provide feedback and contribute to business planning.

* EMIS Knowledge

* NVQ Level 2 or 3 in Health and Social Care or equivalent
* Proficient in Microsoft Office and data collection
* Excellent communication and record‑keeping skills
* Excellent time‑keeping, prioritisation and task completion skills

* Good team relationships
* Understanding of confidentiality, safeguarding, information governance and health and safety
* Experience in a general practice or PCN setting (desirable)
Disclose and Barring Service Check
Required as per the Rehabilitation of Offenders Act (Exceptions Order) 1975. A submission for DBS (formerly CRB) is necessary to check for any previous criminal convictions.
UK Professional Registration
Applicants must have a current UK professional registration.
Other Details
Flexible hours, less than full‑time work considered. The position is mainly based at Hartshill Health Centre.
