North London NHS Foundation Trust is seeking a Mental Health Nurse (Care Co‑ordinator) for HMP Pentonville, North London.
The role supports the delivery of care within the New Models of Care, providing specialist advice to GP and other staff, and coordinating care for prisoners with mental health problems and upon release.
Responsibilities
* Work as a member of the Mental Health Service at HMP Pentonville.
* Provide services across early days in custody, planned care, crisis care and release and transfer.
* Implement sound clinical skills in mental health practice, including CPA care co‑ordination, group work, risk assessment and management, and crisis intervention.
* Manage own caseload as directed by the Mental Health Hub Manager.
* Conduct skilled assessments of patients’ emotional and psychological needs based upon complex data.
* Offer psychosocial interventions for patients with moderate to complex mental health difficulties.
* Jointly facilitate group work with multi‑disciplinary colleagues including education staff.
* Complete clinical risk assessments at first and subsequent assessment interviews and record and report key risks.
* Develop care plans in collaboration with patients, carers and other relevant persons or agencies.
* Review and evaluate treatment plans at agreed intervals with the multidisciplinary team.
* Act as CPA coordinator for appropriate patients, initiating planning and review under enhanced CPA.
* Act as lead professional for caseloads not under CPA.
Qualifications
Qualified Registered Mental Health Nurse (RMN) with experience in mental health and forensic mental health service provision.
Knowledge and experience of offender care and the handling of patients within the prison environment.
Experience in applying recovery and enablement models to align prisoners with community‑based mental health services upon release.
