Advanced Nurse/Clinical Practitioner (ANP/ACP)

PHL Group has an exciting opportunity for experienced Advanced Clinical Practitioners to join us in both our Out of Hours service and triage.

As an Advanced Clinical Practitioner, you will be accountable for delivering safe patient care to a high standard. You will need to be confident in working independently and autonomously at an advanced level of practice across our services, including remote telephone triage and face-to-face consultations in primary or urgent care clinics. Your duties will include all aspects of the patient journey, such as initial history taking, clinical assessment, diagnosis and treatment, prescribing medication, and evaluating patient care. Advanced practice will include evidence of working within the four pillars: clinical, education, research, and leadership.

Main duties of the job

* Assess undifferentiated patients (of all ages from babies to adults) with a range of acute, non-acute, and chronic medical conditions.
* Take patient history, perform appropriate physical examinations, formulate differential diagnoses, and develop/manage treatment plans in any clinical environment.
* Prescribe medication within your clinical competence as required.
* Recognise the need for and initiate emergency treatment to save lives or safeguard patient interests, referring to emergency services when necessary.
* Request investigations and interpret pathology and radiology results to ensure appropriate treatment and onward referrals.

We operate the Integrated Urgent Care service, serving 1.7 million patients across Hampshire, offering telephone assessments, out-of-hours home visits, and face-to-face primary care appointments. Our organisation benefits from a strong infrastructure and clinical leadership.


Job responsibilities

Work autonomously and be accountable for your professional actions.

Assess patients of all ages with various medical conditions, take histories, perform examinations, and develop treatment plans.

Refer patients to appropriate services, including primary, community, secondary, and tertiary care, such as 2-week wait referrals.

Request and interpret investigations, prescribe medications, and initiate emergency treatments when necessary.

Recognise social needs and safeguarding concerns, act accordingly, and ensure safe discharge with proper instructions.

Promote health education and support continuous professional development.

Work within the professional Code of Conduct, ensure accurate clinical records, and support team development.

Adopt a person-centred approach and contribute to developing new service models.

Adhere to KPIs without compromising patient safety.

Maintain clinical competence and comply with organisational policies and legal responsibilities.


Person Specification


Experience

* At least 3 years in a primary care GP practice.
* At least 3 years in an urgent care setting.
* Minimum of 6 years post-qualification experience.
* Competent in managing minor injuries and illnesses.
* Experience working autonomously and within multidisciplinary teams, including mentoring.
* Good knowledge of primary care referral pathways.


Qualifications

* Registered with NMC/HCPC.
* Non-medical prescriber.
* Completed MSc in Advanced Practice or equivalent.
* Minor illness and injury qualifications.
* Teaching and mentorship qualifications.
* Level 3 safeguarding for adults and children.
* Demonstrated competencies in mandatory training.
* Ability to pass an enhanced DBS check.
* Experience interpreting lab results.


Disclosure and Barring Service Check

This role requires a DBS check in accordance with the Rehabilitation of Offenders Act (Exceptions Order) 1975.

Salary: £33.90 to £54.23 per hour, depending on shift patterns.
