Responsibilities
* Case manage children with chronic or acute nursing needs outside the hospital environment within a nurse‑led team.
* Provide highly skilled, quality clinical and non‑clinical care focused on outcomes for children and families, supporting them to live, participate, and achieve in all aspects of life.
* Support junior team members through skill development and completion of staff appraisals.
* Teach parents, carers, and other health professionals as required, and mentor pre‑registered and post‑registered students.
* Collaborate tightly with clinical nurse specialists and Consultant Paediatricians to plan, implement and evaluate patient care.
* Work within a 9‑5, seven days a week service, full‑time 37.5 hours per week, with some weekend and bank holiday duties.
Qualifications
* Registered Children’s Nurse (state registration required).
* Motivated, forward‑thinking, with strong professional knowledge and excellent communication skills.
* Ability to prioritise and work independently as well as part of a team.
* Experience caring for children in community settings is desirable.
