Are you an experienced RMN looking to advance and diversify your career as a nurse? We are looking to recruit a highly skilled, motivated and passionate Band 6 Memory Service Nurse to join our Memory Assessment Service which sits within the Harrow Older Peoples Community Services and provides a specialist service for older people, treating and supporting clients with memory impairment and complex mental healthneeds.

This is an exciting and diverse role for an individual who is passionate about providing excellent quality nursing care. You will have the privilege to influence and shape the provision of high-quality mental health care to some of society's most vulnerable members.

Main duties of the job

The post holder will work as a member of a Harrow Older People and Healthy Aging Service, delivering community mental health assessment and intervention. To provide high quality initial assessment and carry out investigations for Older adults with memory problems, leading to diagnosis as well as reviewing people suffering from memory difficulties.

Harrow Memory Service Team focus: The post holder as part of an integrated, multi-professional team and will assess and undertake a review role for a defined caseload of clients with memory problems.

The post holder will be able to triage, assess and develop a dedicated collaborative care plan that includes signposting to the appropriate support services and agencies for the clients and carers to enable them to plan the future and be responsible for reviewing this in accordance with the service spec.

MAIN DUTIES:

1. To be responsible for the flexible management of a defined caseload designed to meet the individual needs of the client and the carer’s.

2. To maintain high standard of individualized client care, and give guidance on how clients can improve their mental, emotional and physical well-being of clients/carers.

3. To provide a specialist and holistic assessment of client’s memory and initiate any further investigations in the absence of the Clinical Lead and partake in the multidisciplinary team meeting.

4. To develop and maintain a high standard of practice

5. Supervising, monitoring and administering medication as necessary, ensuring the policy for carriage and administration of drugs is strictly adhered to. (RMN/RGN’s only)

6. Monitoring compliance and observing effects and side effects of medication, regularly updating knowledge on medication.

7. Ensure that all views of Service Users are effectively sought, channelled and acted upon, including the efficient actioning of complaints in accordance with the Trust policy.

8. To act in a manner to raise standards of care and make the services culturally sensitive, relevant and accessible to meet the needs of a diverse population.

9. To be conversant with the Safeguarding Adults procedures and alert to signs of possible abuse.
