Overview
Brooklands Health Centre is looking for an experienced Advanced Clinical Practitioner to join our multi-disciplinary team, to help us transform and modernise pathways of care, enabling the safe and effective sharing of skills across traditional professional boundaries. We are currently delivering care services in Milton Keynes from 3 different general practices: Whitehouse, Neath Hill and Brooklands, collectively known as OneMK PCN.
Job responsibilities
In addition to standard nursing duties, the advanced clinical practitioner (ACP) role is accountable in conjunction with the lead nurse for the clinical governance of the practice and is designed to provide clinical leadership in the practice, in order to ensure the safe, effective and compliant delivery of services to all patients. The successful candidate will demonstrate flexibility in attitude and behaviour. They will be keen to undertake new ways of working and delivering healthcare, committing to undertaking the professional development required to adapt to changing circumstances.
Among others, the Advanced Clinical Practitioner is expected to:
* Be registered with current NMC/ HCPC/GPhC registration
* Have a MSc in Advanced Practice or equivalent qualification
* Be a certified Independent Prescriber
* Have primary care experience as well as experience in minor illness management
* Be proficient in managing whole episodes of care, whilst being part of a larger MDT
* Strong communication skills, patience and friendly attitude
* Ability to perform in stressful and complex situation
* Sound clinical reasoning and decision making
* Flexibility
* Ability to inspire and lead a team

Location
Brooklands Health Centre is located in the Milton Keynes suburb of Broughton. It is well connected to transport links and is near bus stops; easy access to the City Centre and the M1.
