Who You Are We are looking for an enthusiastic individual to join our hospital team. What the job involves Your role involves specializing in multi-disciplinary assessments within health settings, specifically managing daily discharges via the D2A pathway and Pathway 3 placements. You will triage cases in conjunction with the Hillingdon HUB and out of borough hospital referrals, attend conference calls with health professionals, supervise staff, allocate cases, and manage stepdown beds. Additionally, you will take on the role of Safeguarding Manager, attend weekly meetings with the safeguarding lead on the progression of investigations, and carry a small caseload of complex cases.
