We are looking for an experienced and motivated Practice Nurse to join our Primary Care Network (PCN) team. This is a key role within our multidisciplinary team, delivering high-quality nursing care to patients across multiple GP practices. Youll be responsible for a range of clinical services including cervical screening, childhood and travel immunisations, wound care, chronic disease management (e.g. diabetes, asthma, CHD), health promotion, and supporting screening and vaccination programmes. Youll also be involved in reviewing patients at risk of long-term conditions and delivering well woman and well man clinics. Were looking for a Registered Nurse with primary care experience. Skills in areas such as venepuncture, ECGs, cytology, and immunisations are essential. Youll need to be confident using EMIS and have a sound understanding of evidence-based practice and clinical governance. You should be able to work independently, manage your time well, and communicate clearly with both patients and colleagues.
