Enablement Worker x 2 posts at 31.5 hours per week
An exciting opportunity has arisen for 2 Enablement Workers within the Inverness Care At Home Team. These are 31.5 hrs per week posts which will enable the right candidates the opportunity to be part of a busy Reablement and Care at Home team. The Inverness Reablement team, provide an important and valued service focusing on assessments at home swift discharges to the residents of the Inverness area. The post will assist with assessing and utilising an enablement approach identifying outcome plans to individuals and their families/carers. The focus will be on maximising independence and supporting the individual to remain in their own home, facilitating prompt discharge from hospital and supporting anticipatory care planning to prevent readmission or admission to a hospital. Requirement to be registered with the Scottish Social Service Council within 3 months of appointment.
