Are you passionate about patient and family care? An exciting opportunity is available to join the Highly Specialised Neuromuscular Team based at the International Centre for Life as a Regional Care Advisor. This is an established service with close working links to the John Walton Muscular Dystrophy Research Centre delivering care to patients and their families with complex care needs across the Region.
You’ll be part of an ambitious, innovative and passionate multi‑disciplinary clinical team balancing medical management with a social model of disability management, identifying needs and providing guidance as early as possible to enable best outcomes, support and care pathways for our patients.
As a Regional Care Advisor you will be an Allied Health Care Professional, or Nurse Registered with the appropriate body, skilled and experienced in working with patients with complex health conditions, initiating evidence‑based practice & developing care guidelines to address the health and social care needs of patients and their families.
Main duties of the job
* Work as an autonomous practitioner delivering specialist expertise and advice to families and individuals living with neuromuscular conditions on options available to them
* Lead in the care and education of patients and carers about living with a neuromuscular condition
* Act as an advocate for families and individuals in their interactions with other professional staff and services
* Act as an educational resource for other agencies in the region involved in delivery of care
* Work within the multi‑disciplinary Muscle Team in developing and delivering care pathways for individuals/families with neuromuscular conditions
Job responsibilities
* Advise families and individuals regarding the living options available to them, balancing medical management with a social model of disability management
* Lead in care and education of patients and carers
* Act as an advocate for families and individuals in their interactions with other professional staff and services
* Exercise high levels of judgement, discretion and decision‑making in the clinical care of children, young people and adults with neuromuscular conditions within a defined geographical area
* Work as an autonomous practitioner delivering specialist expertise and advice
* Act as a highly specialist/educational resource for all other agencies in the region involved in delivery of care
* Contribute to the development of the multi‑disciplinary team objectives and care pathways for the Muscle Team
* Contribute to local clinical networks both regional and national, and speciality specific groups working in partnership with other key professionals
* Identify gaps in service provision and implement changes to improve service delivery
* Promote client equality, maintaining privacy and dignity, promoting individual diversity and rights
* Identify local, demographic, cultural and environmental factors that influence the health and quality of life for this client group
* Ensure all patient records are maintained in line with Trust policy
Person Specification
Qualifications & Education
* Allied Health Care Professional (e.g. Occupational Therapist or Physiotherapist) or Nurse registered with the appropriate professional body
* Evidence of continuing professional development and the development of others
Knowledge & Experience
* Significant experience to MSc equivalent level of working with patients with complex health conditions initiating evidence‑based practice and developing care guidelines
* Working knowledge of the psychosocial impacts of complex health and/or neurodevelopmental conditions
* Working knowledge of support available to include health and social care, Education, Health and Care Plans (EHCPs)
Skills & Abilities
* Ability to work across health and community settings, with a variety of statutory and non‑statutory service providers
* Ability to collaborate in a multidisciplinary team to advise and guide other professionals based on known expertise whilst also taking on board other contributions
* Ability to act as a catalyst for change, challenging professional and organisational boundaries, generating alternative solutions which best meet the needs of the client group and their families/carers
* Exercise high levels of judgement, discretion and decision‑making in the clinical care of children, young people and/or adults
* Ability to manage safeguarding issues effectively
* Ability to demonstrate tact, diplomacy and empathy with colleagues, patients and relatives
* Strong time management skills with the ability to prioritise effectively
* Excellent record keeping and report writing in line with Trust procedures and Professional Code of Practice
