Overview
An exciting opportunity has arisen within the Patient Flow team. The role involves working across the Multi‑Disciplinary Team within acute and community services. The post is pivotal to the flow of patients through the healthcare system, with the post holder being the driving force and coordinator of services that promote timely discharge from hospital to home or other care settings throughout the region of Dumfries and Galloway.
Key Responsibilities
* Coordinate and manage the flow of patients across acute and community services.
* Work with the Multi‑Disciplinary Team to ensure timely discharge and transfer of care.
* Promote efficient patient movement and improve service integration across the region.
* Facilitate communication between acute hospital settings and community care providers.
