Overview
We have an exciting opportunity for a community CNS position at Meadow House Hospice to help deliver the new North West London model of specialist palliative care. The model increases visiting hours for patients, improving timely access to care. The hospice provides specialist palliative care to residents of Ealing and Hounslow.
Key Responsibilities
* Provide clinical care for community patients with complex palliative needs, including assessment, history taking and managing a patient caseload.
* Collaborate with primary care teams, allied health professionals and other healthcare providers.
* Exercise a high degree of professional autonomy and make critical judgments to meet the role’s demands.
* Lead, innovate and motivate the community team.
Qualifications and Experience
* Qualified nurse with experience in palliative or community care.
* Strong clinical skills and ability to manage complex patient needs with compassion and efficiency.
* Excellent communication, interpersonal and interpersonal skills.
* Experience in working with primary care teams is an asset.
