Job Responsibilities
* Provide support and opportunities to enhance the life and social skills of adults with learning and physical disabilities, as well as complex health needs.
* Implement and deliver activities from community-based hubs to help achieve individual outcomes and support both physical and emotional needs.
* Assist individuals with personal care, hydration, nutrition, and medication while adhering to safeguarding procedures.
* Develop, design, and maintain resources for activities with a person-centered approach.
* Observe, monitor, and promote the health and well-being of individuals both in services and within the community.
* Contribute to care and support plans, highlighting changes that may require updates.
* Follow financial procedures, ensuring best value for money, and maintain written records for work-related purchases.
* Encourage independence in managing finances among individuals supported.
* Adhere to policies regarding the recording, sharing, and storage of personal information.
* Maintain positive professional relationships with colleagues, families, carers, volunteers, and other stakeholders.
* Participate in various meetings, including supervision, team, and review meetings.
* Follow Health and Safety policies, including food hygiene, first aid, and incident reporting, and report issues promptly.
* Maintain a high standard of housekeeping within day care settings and external grounds.
* Comply with building inspections and security procedures, reporting faults or repairs.
* Prioritize daily workload effectively.
* Mentor and support new staff, volunteers, and student placements.
* Contribute to fundraising and social events within the service and local communities.
* Assist with ad hoc tasks to support coordinators and management.
* Adhere to the Staff Code of Conduct, maintaining confidentiality and dignity in care.
Person Specifications
* Knowledge of person-centered approaches for delivering care and support to adults with disabilities.
* Experience in providing care and support in social care settings, whether through employment, voluntary work, or life experience.
* Understanding of health and safety legislation and compliance with relevant policies.
* Effective communication skills with colleagues, families, carers, and individuals supported, both verbally and in writing.
* Desirable: Health and Social Care Qualification Level 2 or above, or willingness to work towards it.
* Desirable: Experience supporting adults with behaviors of concern and complex health needs.
