About The Role

We have an exciting and rewarding opportunity to join our team as a Dementia Adviser, working within the Dementia Wellbeing Service in Bristol. This is an integral and trusted partnership which brings together the NHS and the Alzheimer’s Society. Our Dementia Advisers are part of a vital multi-disciplinary team to deliver dementia guidance, support and advice across the city, allowing the successful individual to make a real difference in the lives of those affected by dementia. We strive to help maintain independence, improving sense of well-being, and putting people in more control of their own lives. We promote a person-centred service so that our support is unique to everyone, based upon their own aspirations and needs. Location: Community based in Bristol with some homeworking. Contract: Fixed term contract - 12 months. Hours: 21 per week Interview: 10 December via Teams

You are:
1. Passionate about supporting people to maintain their independence, improve their sense of well-being.
2. Possess professional or personal experience of supporting people with additional care requirements.
3. Able to network effectively and collaborate with other professionals to achieve positive outcomes for people.
4. Capable of managing a caseload of service users effectively and independently, ensuring face to face reviews and telephone contacts happen within expected timescales.
5. Have strong written communication skills and can make accurate and confidential records.
What you’ll focus on:
6. Managing an existing caseload of service users as well as new referrals to deliver ongoing post-diagnostic support.
7. Working collaboratively with NHS clinicians to ensure the right support is offered to people at the right time.
8. Writing and updating of ‘Wellbeing Plans’ that capture a person’s history, current support needs, and future wishes.
9. Supporting our peer-support groups, delivering carers education courses and engaging in awareness raising events in the community.
10. Identifying service user support needs and referring onwards to appropriate organisations (e.g. social services, other charity organisations).
