GP with Extended Role (GPwER) in Gastroenterology training programme

The advert is for a GP with Extended Role (GPwER) in Gastroenterology training programme to join 9 consultants.

The successful candidates will be encouraged to develop their sub‑specialist areas of interest.

The work is based at Kingston Hospital, with clinics taking place in the main outpatient department and outpatient endoscopy which has recently moved to a brand‑new, state‑of‑the‑art endoscopy unit within the main hospital site. At present some outpatient consultations are performed remotely by telephone with adequate working space and IT support provided.


Main duties of the job

* 1. Join MDT performing enhanced clinical triage of referrals – All referrals are reviewed via an enhanced virtual MDT on Microsoft Teams. This includes outpatient referrals from local GPs, A&E referrals and patient‑initiated follow‑ups. The MDT comprises Consultant Physicians, specialist nurses and the GPwER.
* 2. Supervised clinics in secondary care and in community hub(s) – This will include general gastroenterology clinics and clinics based on endoscopy (there are also plans to start a one‑stop dyspepsia/endoscopy service). The community clinics will be closely linked to a community diagnostic hub programme to enable the review of patients post‑diagnostics such as fibroscan, CT, TNE. This can start off as working with the consultant initially, and then gradually the GPwER will build their own clinic and patient selection.
* 3. Educational & interface work with General Practice + SWL Gastroenterology Pathway Development – (a) Upskilling and educating clinicians in primary care (optimising use of A&E, clinical guidelines on management of non‑ulcer dyspepsia etc.) as well as raising awareness of referral pathways. (b) Working with the SWL Gastroenterology Clinical Network Lead on network transformation priorities in gastroenterology for SWL.

Job responsibilities

Patient empowerment and community activation

Work with underserved communities and areas of deprivation (core20+5) to raise awareness of common gastroenterology conditions and cancer, including when to contact health services. For example, alert symptoms for gastric cancer etc. This will be coordinated through Merton PCNs partnership with Merton Connected (the 3rd sector organisation for Merton with whom Merton PCNs have an MOU).

Mentorship support will be provided.

Person Specification

* Full GMC Registration with Licence to Practice (or eligibility to obtain). RCGP or equivalent.
* Attended management training course. Knowledge of finance/budgets.
* Evidence of continuing medical education.
* Demonstrable commitment and ability to teach and train undergraduates and junior doctors. Experience of clinical audit. Relevant research articles in peer‑reviewed journals.
* Able to communicate at a strategic and operational level.
* Appropriate higher degree (e.g. MD, PhD or equivalent).
