An exciting opportunity has arisen for an experienced HCSW to join our Intermediate Care Team as part of the Integrated Community & Support Services (ICASS).
The Intermediate Care Service provides rehabilitation to the adult population within the locality. This will be delivered to service users in their own homes or as out‑patients.
The primary function of this role will be to work with the Physiotherapy Service within Intermediate Care to deliver therapeutic treatment programmes to service users in their own homes, preventing hospital admission and supporting early discharge from hospital. Therapists will provide specialist assessments to this patient group and allocate work to this post holder. The post holder will also be expected to hold their own caseload, assess, progress and discharge autonomously within parameters.
Qualifications required include an HNC in Health and Social Care or equivalent, or SCQF level 8 (e.g., Postural Stability Instructor) or equivalent. Experience working in the NHS as a therapy support worker is essential, and experience working with older people is also essential.
Key responsibilities and expectations:
* Work as part of a supportive multidisciplinary team to carry out ongoing treatment programmes for a mixed caseload of mainly older people.
* Work independently and within the team; maintain excellent organisational and communication skills.
* May be required to work in one of the other community hospital sites as occasion arises.
* Be able to travel throughout the area; travel is essential for this community post.
* Public holiday working is in operation.
* Participate in regular supervision, PDP and training opportunities.
