We have an exciting opportunity for a clinical pharmacist to join our friendly practice based in Oxfordshire.
You will be working solely for Hedena Health and join our existing team of two pharmacists. Main duties of the job
The pharmacist will work as part of the multidisciplinary team, including:
* Assisting patients with medication-related problems and queries
* Supporting general practice staff with repeat prescriptions and medication queries
* Collaborating with the prescribing team on safety and quality of prescribing
* Conducting medicines reconciliation and reviewing repeat medications for newly registered patients
* Supporting training such as independent prescribing, if not already qualified
* Developing skills in areas of interest like respiratory, diabetes, or minor illness
* Carrying out structured medication reviews
* Undertaking clinical audits to enhance patient care and safety
* Participating in contract-related work to meet targets for QOF, IIF, and prescribing incentives
* Assisting with chronic disease management and reviews, such as hypertension, diabetes, CKD, and hypercholesterolaemia, depending on experience and training

Job responsibilities
Role Outline
The pharmacist will work within the multidisciplinary team in:
* Patient-facing roles—assisting with medication-related problems and queries
* Staff-facing roles—supporting practice staff with repeat prescriptions and medication queries
* Prescribing team roles—working on safety and quality of prescribing
Practice supervision
* Monthly supervision sessions with the senior clinical pharmacist
* Day-to-day supervision by the senior clinical pharmacist or acute hub practitioner (if trained in minor illness)
* Additional support and training from the GP line manager
Specific Duties
The working day is 9 hours, including 30 minutes for lunch and two 15-minute coffee breaks. The schedule is:
* Morning: 08:30 to 13:00
* Afternoon: 13:30 to 17:30
Typical structure includes:
* Two hours for repeat prescription requests and routine appointments or blood test reviews
* Dedicated sessions for chronic disease reviews, e.g., diabetes
* One session per week for contracts work and audits
* Sessions for acute illness if trained, supervised by an emergency practitioner
* Ad hoc meetings with the prescribing lead

Experience
* Proven experience in primary care
Qualifications
* Bachelor's degree in Pharmacy, registered with the General Pharmaceutical Council
* CPPE or equivalent training or pathway
