Overview
Do you want to make a real and valuable difference to patients in their own homes? Would you like to join a service that provides a comprehensive nursing induction programme and clinical supervision, with the opportunity to lead a team of nursing staff in the community? This role is for a registered nurse or paramedic who will lead a Neighbourhood Nursing Team in Strood, within a locality, providing high-quality care to housebound patients aged 18 and over in the Medway area.
Due to the nature of the role, you will need a full UK driving license and access to a vehicle.
Responsibilities
* Lead and manage a Neighbourhood Nursing Team in Strood, ensuring a high standard of nursing care to patients and their families in the community.
* Assess, manage, plan and deliver care for patients allocated to you, being professionally and legally accountable.
* Collaborate with GPs, other professionals and statutory and voluntary agencies; work within and across professional and organisational boundaries as part of an integrated health care team.
* Plan, prioritise and organise workload to meet service needs; provide care within the local health care system and contribute to timely discharge and admission avoidance where appropriate.
* Cover the service across the shifts: 7am to 3pm, 8.30am to 4.30pm and 11am to 7pm; the service operates 7am to 7pm, 365 days a year.
Qualifications and Experience
* Recognised degree in adult nursing; Registered Nurse on Part 1 of the Nursing and Midwifery Council (NMC) register.
* District Nursing qualification / community practitioner pathway or willingness to commence within 1 year of starting; Nurse prescriber or willingness to undertake training; relevant post-graduate qualification (e.g., elderly care, chronic disease management, long-term conditions, rehabilitation).
* Postgraduate experience in elderly/rehabilitation/community/long-term conditions and in autonomous practitioner roles; experience in community healthcare and high-level clinical assessment of complex patients.
* Experience of using technology to facilitate patient care; motivational interviewing; change management; mentoring students and other healthcare professionals; caseload management.
* Intermediate IT skills (Windows, Internet, Outlook, Excel); presentation/teaching experience; research awareness and audit skills.
* Special knowledge in community nursing and long-term conditions; knowledge of clinical governance and risk management; ability to practice at an advanced level; experience of lone working and decision making.
* Disposition: self-motivated, autonomous, patient, compassionate, inclusive; flexible and able to adapt to service needs; capable of recognising personal limitations and referring appropriately.
* Excellent verbal and written communication; effective time management and organisational skills; ability to travel to dispersed sites.

* We are a small social enterprise providing NHS services. Overseas nursing candidates who do not have full UK registration with the NMC cannot be shortlisted. NMC PIN must be provided on application.
* We cannot support OSCE preparation, OSCE exam, accommodation or pastoral care.
